I think Scott is absolutely correct. The Wells plan only shows first class areas, and since that's a border, there was no detail drawn. The thin white line is misleading.
Anyway, there is (almost) photo proof that the door was there. Check out the Shipbuilder book. In the chapter describing Titanic that is from a later issue, figure 3 (different page # depending on the edition you have) there is a rendering of the Cafe looking aft. A door is clearly seen on the aft wall.
